Louisiana-based healthcare provider, Lafourche Medical Group, has been fined $480,000 by the Office for Civil Rights (OCR) for a HIPAA violation, following a data breach affecting almost 35,000 patients. The breach exposed patient data including addresses and lab test results. The incident, caused by a phishing attack, revealed that Lafourche failed to adhere to the HIPAA Security Rule, neglected to conduct risk analysis and system reviews.
