OSF deploys care transition program, reduces readmission rate from 29% to 9%

OSF HealthCare, a provider in Illinois and Michigan, has successfully reduced hospital readmission rates by using virtual care management and patient monitoring systems. The technology allows staff to track patient health data in real time and intervene before patients require hospital treatment, resulting in reduced readmissions. The system is part of a comprehensive care model which encourages renewed collaboration between Skilled Nursing Facilities and home care providers to enhance patient outcomes. Since implementation, OSF Healthcare’s readmission rate has dropped from 29% to 9%.
