Organizational accountability in medical errors

The healthcare system should adopt a Just Culture approach, looking beyond individual blame and assessing patient safety incidents in the context of system and situational factors. Errors are often caused by inadequate support, resources, learning systems and standardized protocols, rather than individual negligence. When assessing incidents, healthcare providers should also consider their responsibility to produce an outcome, follow protocols, and avoid harm. Implementing systems that prompt providers about allergies and other critical information can help in preventing such incidents.
