Social determinants of health (SDOH), such as food, shelter, and security, contribute to a high risk of disease and hospital readmissions among vulnerable populations. These factors can account for 80% to 90% of health outcomes compared to medical care alone. An opportunity to improve healthcare outcomes exists in incorporating non-traditional health data into patient records, but this process is challenged by lack of interoperability and awareness of SDOH data.
