Health System Pays Feds 0K to Settle HIPAA Breach Case

California healthcare network PIH Health has agreed to pay a $600,000 fine and implement a corrective action plan after a 2019 phishing breach affecting nearly 190,000 people. The company violated HIPAA rules by not reporting the breach within 60 days and failing to use or disclose protected health information as required under the Privacy Rule. The fine is part of the 12th HIPAA enforcement action of 2025 by HHS OCR.
