The Georgia Health Information Network has developed Georgia Unity, a health information exchange network aimed at understanding total health in rural areas. The service aims to utilise social determinants of health to enhance health equity. The program uses Aetna’s health platform to provide referrals to community-based partners for high-need individuals. The scheme’s first year saw savings of $11,248 per individual annually, with 2,000 referrals made to health resources in Emanuel County.
