The service aims to utilise social determinants of health to enhance health equity. The program uses Aetna’s health platform to provide referrals to community-based partners for high-need individuals. The scheme’s first year saw savings of $11,248 per individual annually, with 2,000 referrals made to health resources in Emanuel County.
