Pennsylvania’s Department of Human Services will collaborate with Pennsylvania’s Consortium of Health Information Exchanges and Findhelp to create an online tool offering comprehensive social care services. Called PA Navigate, the tool aims to provide ease and privacy in assessing patient’s social care needs. The initiative will allow residents and various service providers to source and keep track of referral services catering to specific health needs.
