Providence health system in Washington has expanded into remote patient monitoring (RPM) which it sees as a viable solution to improve patient care and outcomes and alleviate some of the pressures faced by PCPs and care teams, notably for patients with chronic conditions. The RPM technology, provided by Cadence, enables team-based care, allowing clinicians to monitor a patient’s health from their home and intervene if necessary, ultimately reducing hospital visits and improving efficiency. The program is anticipated to reach 5,000 to 8,000 patients by end 2024.
